Get yourself tested! Free gonorrhea and chlamydia testing will be offered Fall and Spring semesters.

By appointment only—you must call 706-507-8620 to schedule. (Appointment times fill quickly!) Paperwork is required and can be completed online (PDF) prior your appointment. Once it is complete, send it back to: student_health_services@columbusstate.edu.

Note: If you have symptoms or concerned about exposure to an STI, you may need to speak with the nurse prior to scheduling.

About Us

The Student Health Center (SHC), located on the Main Campus in Tucker Hall, offers affordable health care to all currently enrolled CSU students who have paid the Student Health Fee. SHC services are available by appointment. Please wear a mask in the clinic if you are feeling ill. Our staff includes registered nurses, a nurse practitioner who specializes in women’s health, and 2 family practice physicians who are available on a scheduled basis.

Your Student Health fee affords you 4 visits with a provider each semester. If you require more than 4 visits, there may be an additional fee per visit. Our nurse visits are unlimited, at no additional cost.

Parents

The SHC understands that you may worry about your student’s health and well-being, and we encourage you to have an open relationship with your student when it comes to their healthcare. It is helpful for your student to know their personal health history, such as allergies to food or medications, and your family history. However, as health care providers, we must follow FERPA, HIPAA, and Patient’s Rights guidelines. This means the SHC is unable to discuss any information regarding your son or daughter, if they are 18 years or older, without their written authorization for release of medical information (PDF)

We ask you to please refer to the HIPAA and Privacy Guidelines listed under the Forms tab, to the left of this page, for any clarification on this matter. However, feel free to contact us if you have general questions or if you have information to share with us regarding your child.

The Student Health Insurance Plan (SHIP) provider, United Healthcare Student Resources (UHCSR) is available for purchase. It also has telehealth resources. Please see Student Insurance for more detailed information.


Location

The Student Health Center is located in Tucker Hall on the west side of campus between Richards Hall and Arnold Hall. Parking is available in student permitted spaces. The closest Parking Lots are 3, 5, 7.

There is no student parking available in the lot connected to Tucker Hall at this time.

Hours

The SHC will take appointments:

  • Monday – Thursday 8:30 a.m. – 4:30 p.m.
  • Friday 8:30 a.m. – 2:00 p.m.
  • Closed for all official University closures. See the Academic Calendar.

Smoke-Free Campus

Effective October 1, 2014, BOR policy prohibits the use of all forms of tobacco products (including vaping) on campus property. This policy applies to all employees, students, contractors, and visitors and is applicable 24 hours a day, seven days a week.
